b6c1e1133d Add z-index: -1 to select down arrow 
Without this change clicking exactly on the down arrow eats the click and
doesn't active the <select> element.

af24efddaa Style the Artwork editing form the same as the creation form 
It's a complicated selector because it also must exclude the PATCH form on `artworks/get.php`, which has similar attributes.

Moreover, the browse form on `artworks/index.php` also has similar attributes.

8499cfeaad Make sure that headers scrolled in view on mobile 
Currently, when clicking on a ToC link in a single page web version, the title for that section is hidden under the fixed banner. We can use `scroll-margin-top` to add a little additional scroll amount to scroll the header into view. `3em` matches the current top margin of `3em` on headings and hgroups.
